She serves the dog meals with this new generation nano vibrating cells that make the shit, when it comes out, to vibrate like a cell phone - so she hears the shit. Easy, then, for her, to know exactly where it is. Simple as that

Guide dogs are trained to go on command, they are also trained to not to cock their leg when they pee or freak out if someone's touching them while their going.
The handler just tells them when to go, they then feel down the ridge of their back to determine how the dog is standing and if it's peeing or pooing they then know what it's doing and a fairly good idea of where it's doing it too.

Obviously some people who have guide dogs are only partially sighted so they just look like the rest of us ;)
